Resection

The removal of the inflamed tissue of the root apex and the cleansing of the environment. It’s possible to save the tooth that otherwise would have to be removed.

  • its indication: The root canal filling that doesn’t reach the top but it fills up the root canal to a certain level. A root canal instrument is broken into one of the root canals. Anatomical causes e.g. if it’s not possible to put root canal filling into a very crooked root canal or if the root canal is impermeable. Overfilled tooth that causes constant problems. The inflammation must be removed at the incisor or premolars because of focal infection.
  • what happens prior operation: The treatment will be preceded by a dental examination. An x-ray will be taken of the tooth and the treatment, the type of local anesthesia, the healing process and possible complications will be explained. If there is no root canal filling in the tooth then we perform the root canal treatment first, then after 24 hours the operation can take place. It is advised to eat before every dental treatment, because it is recommended not to eat until the numbness lasts, which is usually for 3-4 hours after the injection.
  • steps: The treatment is done in local anesthesia. A gingival flap is made by cutting the gum, then a little hole is drilled into the bone around the root apex. The diameter of the hole is as big as the size of the infected area. One third of the root apex and the inflamed tissue are removed. The root canal filling at the end of the root apex is closed down with dental cement. The edge of the bone in the wound is smoothed, the gingival flap is placed back and it’s closed with stiches.
  • potential complications: The inflammation around the root apex will not heal and another periapical abscess appears. That’s why the healing and ossification processes must be monitored and a control is necessary in every 6 months. The gum can be damaged, the labial angle can crack,and herpes can appear on the lips. The face can swell up at the area of the operation.
  • what to do after operation: It is forbidden to suck on the wound or to rinse the mouth with water as it can cause bleeding. From the following day the wound must be cleaned very carefully with a soft brush. Fomentation, warming it with pillow is forbidden. The swelling must be chilled with a cold pack from the outside! Consumption of milk or dairy products is not advised for a few days. In case an upper resection was performed for a few days avoid nose blowing, hard physical work and bending. Take painkillers if necessary and antibiotics only how your doctor prescribes it. The stiches will be removed in 5-7 days.
